Presentation A 25 year old male, presents with a one day history of acute left-sided scrotal pain. He has no history of trauma.
Caption: Transverse view of the epididymis on both sides.
Description: The left epididymal body and tail are enlarged and have a heterogeneously hypoechoic appearance. The right side is normal.

Description: The left epididymis is very hypervascular as compared to the right side, consistent with an inflammatory pathology.
