Presentation This is a transrectal scan in a patient with carcinoma of the lung, who presented with elevated PSA levels.

Description: The left seminal vesicle shows the presence of at least 3 well-defined, rounded hypoechoic masses. His previous scans were normal. In view of his clinical condition and elevated PSA levels, metastases was the most likely diagnosis.

Follow Up A transrectal ultrasound guided biopsy was performed and histopathology confirmed metastases to the seminal vesicles.
