Right parathyroid adenoma

Presentation A young man presents with painless swelling on the right side of the neck of 15 days duration. An ultrasound of the neck was performed.

Description: A well-defined, oval, hypoechoic solid mass is seen posterior to the right lobe of thyroid and medial to the carotid. The mass has a homogeneous echotexture and a demarcating capsule, indicating its extra-thyroidal location.

Description: Vessels are demonstrated within the extra-thyroidal mass. This pattern of vascularity is more consistent with a parathyroid adenoma rather than a lymph node which typically demonstrates central hilar flow.

Description: There is increased vascularity in this mass compared to the adjacent thyroid vascularity, which is often seen with parathyroid adenoma.

Final Diagnosis Right parathyroid adenoma
Follow Up Nuclear imaging and histopathology confirmed the diagnosis of a hypercellular parathyroid adenoma.
