Presentation A 45 year old woman with a known history of an ovarian cyst and anterior wall fibroid, presents for follow up ultrasound. Transvaginal images are obtained and observations other than the ovarian cyst were noted.
Caption: Sagittal view of the uterus
Description: The fundal portion of the endometrial canal shows highly echogenic material within it. Dense posterior shadowing is noted in that region.

Description: Hyperechoic material is seen in an elongated tubular fashion, running along the right adnexal region. This corresponds to the course of the right fallopian tube.

Description: A soft tissue mass is seen within the lower uterine segment/cervical canal. There is minimal fluid within the endometrial canal.

Description: The soft tissue mass is seen again in this image within the endometrial canal and shows lobulations. A small amount of fluid is seen in the canal.

Description: No abnormal vascularity is identified within the mass.
Final Diagnosis The hyperechoic material in the endometrial canal of the uterine fundus and within the lumen of the fallopian tube represents silicone that was injected into the tube for the purpose of sterilization. There is a lobulated endometrial polyp in the lower uterine segment.
