Presentation
Primigravida with 20 wks gestation came for a routine scan.
Caption:
Transabdominal and transvaginal scans of lower uterine segment.
Description:
Two lobes of placenta seen, one bigger than the other suggesting succenturiate lobe.
Differential Diagnosis

Discussion
Two lobes of placenta seen, one bigger than the other suggesting succenturiate lobe.
Vessels are seen connecting the two lobes. The vessels are crossing the internal os but not lying over the internal os. They are far from the os (supero-inf distance from the internal os was 2.9 cm) hence it cannot be called a vasa previa. However since the vessels are unsupported there is a high risk of hemorrhage.
